Intake Process:

Visit during program hours and bring all required documents below

Documents Required:
- Photo ID
- Recent utility bill as proof of residency
- Proof is required for the Reason for Assistance: SSI, TANF, Unemployment, LIHEAP, Senior Freeze, Medicaid, and/or Subsidized Housing
- Copy of the most recent Public Aid Medical card
- Social Security cards for all children in household
- Any others living in the home will need to bring in proof of their residency, such as mail received by them at the identified address

**Drivers Licenses are NOT accepted as identification for proof of residency; must bring recent utility bill

-Meats, corn beef hash, stews, tuna, salmon, sardines, chili, spaghetti O!s, ravioli, beefaroni, sloppy joe mix, soups, baked beans, pork & beans, kidney beans, canned vegetables/fruits, jams and jellies/peanut butter.

SAUCES:
-Spaghetti sauce, gravy and sauce mixes.

DRY GOODS:
-Rice, beans, macaroni & cheese, Rice a Roni, Pasta Roni, Tuna Helper, Chicken Helper, Hamburger Helper, crackers, cookies, snack food, spices, cereal, pancake mix and syrup, jellos/puddings, sugar free products

CONDIMENTS:
-Ketchup, mustard, mayonnaise, barbeque sauce, salad dressing, spices

MILK:
-Powdered, evaporated and condensed only

BAKING SUPPLIES:
-Sugar, flour, salt, baking powder, baking soda, vanilla, chocolate chips, nuts, raisins.
