Description:
Provides diapers and other basic needs to families at no cost through community partners as well as direct service. Also offers incontinence supplies for the older adults, people with disabilities, and children with special needs.
*There is no Zion Site for the Mobile Diaper Pantry.*
Eligibility:
Must be an Illinois or Wisconsin resident - services provided for families and children. For incontinence supplies, must be ages 65 years or older or ages 5-64 years old with special needs with a doctor's note/diagnosis.

Documents required:
- Online client intake form
- Government issued photo ID for parent (state ID, driver's license, military ID, passport, VISA)
- Birth certificate for any child in need of assistance
- Prescription of incontinence or IEP/doctor's note with diagnosis (for incontinence supplies)
- MediCare or MediCaid card (for incontinence supplies)
