Description:

Provides supervised residential setting for adults living with severe mental illness. Length of stay can be between 6 - 12 months. Offers residents help to understand their condition, develop coping skills, and improve their interpersonal and daily living skills. Services include:

- Medication Monitoring
- Psychiatric Care
- Linkage to Community Resources & life skills training ( funding, housing, vocational, health care, insurance & entitlements, budget management, cooking, housekeeping, socialization. laundry, personal care & recreation).

Eligibility:

Must be Lake County resident suffering from chronic mental illness. Must have potential to live in a less structured environment and motivated to attend some form of day structure.

Program Fees:

- Sliding scale fee based on income.

* Individuals contribute 60% of their total monthly income (not to exceed $635), for room and board, utilities, phone, transportation and other services.
