Description:
Provides information and referral services to seniors and their caregivers. Offers an assessment of needs and connects them to local resources, such as:
- Home delivered meals or community dining programs
- Benefits programs to help pay for prescription drugs, health care, rent, utilities, and other needs
- Family Caregiver Support for those who are giving unpaid care to adults with Alzheimer!s disease or a related disorder.
- Transportation services (doctor appointments or personal errands)
- In-home assistance ( light housework, personal care, and meal preparation)
- Community engagement activities
Also provide advocacy and follow-up to see that the services were received.
Eligibility:
- Persons 60 years of age or older
- Persons living with disabilities
- Family/informal caregivers of older adults and adults with dementia
- Older relatives providing care to children under age 18 or adults ages 18-59 with disabilities
