Description:

Provides alcohol/drug evaluation to determine the amount of hours of treatment needed for an individual to complete. Offers treatment, information, counseling, and support systems to help modify attitudes and behaviors that contribute to driving under the influence.

Eligibility:

Must be individuals 16 years or older.

Sliding scale fee based on income

- Drivers abstract obtained within the last 30 days from the Secretary of State
- Notice of Summary of Suspension (white sheet of paper issued at the time of the arrest)
- Referral slip from court
- Attorney's name, address, and phone number
