Description:
Provides a full range of mental health services to youth and adults. Services include:
- Individual and group counseling
- Consultations
- Mental Health Evaluations
- Assessments
- Psychotherapy Treatment
Also provides referrals for ongoing psychiatric care treatment and support services.
Eligibility:
Individuals 12 years of age or older.

Sliding scale fee, if no insurance
Intake Process:
Call for an appointment.
Documents required:
- Proof of health insurance, if applicable
- Proof of income, if sliding scale
- Referral provider's pay order, if a referral source is paying for services
