Description:

Provides group homes for individuals with intellectual and/or developmental delays with 24/7 support. Develops a plan with an Aspire Life Consultant to learn independent living skills. Takes part in all aspects of running a home, such as cooking, cleaning and grocery shopping. Weekly outings with a friend, group, or independently is encouraged.

Eligibility:

Must be 21 years or older with a documented disability of Intellectual Development Disability (IDD)

Intake Process:

Referral required from an Independent Service Coordination (ISC) agency at the Illinois Division of of Developmental Disabilities

Documents Required:
- Referral Packet from ISC agency
- Picture ID
- Social security card
- Proof of Medicaid
- Documentation of diagnosis
- Current physical within one year
- Current TB test
