Description:

Alexian Brothers Community Housing Program is permanent supportive housing that utilizes a harm reduction model. Permanent supportive housing is housing that focuses on flexible and comprehensive support services, in addition to affordable, long-term housing. This program serves chronically homeless individuals living with chronic diseases, including HIV/AIDS, throughout Lake County.

We provide case management and supportive services to ensure that our clients receive the support and tools they need to achieve their overall health and wellness goals.

Eligibility:

An applicant for residence must:
1. Have a diagnosis of HIV/AIDS
2. Be chronically homeless
3. Be an individual that is 18+ years or older

Languages:

English, Spanish (limited availability)

Program Fees:

Clients pay 30% of their income as rent, if they have an income (having no income is not a barrier to admission)

Intake Process:

Intakes should be scheduled through our Director of Community Housing, Nora Johnson.
