Offers highly structured housing, independent permanent housing, transitional living environments and aftercare programs.

Transitional housing
The Ascension Illinois Housing and Health Alliance!€™s recovery homes are licensed by the state of Illinois. You!€™ll start by personalizing a plan with an on-site case manager who will meet with you regularly.
A certified alcohol and drug counselor (CADC) provides:
- one-on-one
- group recovery support in a sobriety-based living environment.

The Harbor Waukegan offers fully furnished private rooms, food and nutrition services, 24/7 staffing, a learning center and more.

Aftercare
The first two years after you leave a transitional housing program can be difficult and increase your risk of substance use and homelessness.

Services include:
- Check-ins at least once every three months
- Emergency assistance to help pay for overdue utility bills, security deposits, books and school fees, and medication co-pays (first year after program completion)
- Support for your housing and health needs

Permanent Housing:

Ascension Illinois Community Housing Program Lake County provides:
- Rental subsidies
- Utility allowances (if needed)
- Food vouchers
- Medication copays

Eligibility:

An applicant for residence must:
1. Have a diagnosis of HIV/AIDS
2. Be chronically homeless
3. Be an individual that is 18+ years or older
4. Experiencing no or low income
5. Be willing to participate in case management services

Languages:

English, Spanish (limited availability)

Program Fees:

Clients pay 30% of their income as rent, if they have an income (having no income is not a barrier to admission)
