Description:
Serves as health center with variety of outpatient services. Provides Pediatric Services, Women's Health Services, Primary Care Services, and Dental Services.
Pediatric Health Services:
Provides complete care to children from birth to age 18.
Services include: developmental assessments, hearing and dental screening, basic vision testing, immunizations, and lead testing.
Women's Health Services:
Provides wide range of outpatient comprehensive services for women of all ages. Services include counseling, prenatal, and gynecological care.
Primary Care Services:
Offers broad spectrum of personal and community services directed at individuals and families of all races, genders and ages who are unable to access the private medical sector.
Dental Care Services:
Provides primary dental care for children and adults. Services include: dental exams, cleaning, and referrals to Orthodontics.

Must be Lake County resident

Fees are assessed based on the services needed, with adjustments made depending on the individual or family income.
