Description:

Provides door-to-door transportation for non-emergency medical appointments for older adults. Volunteer drivers transport clients to and from appointments using their personal vehicle. Vehicles can accommodate canes, light walkers, and small oxygen tanks. Provides a vendor transportation service for clients requiring a wheelchair.

Also provides free grocery shopping service for seniors. Provides volunteers to shop and deliver groceries. The client must pay for the groceries.

Eligibility:

- Must be Lake County residents ages 60 years or older
- Service is limited to two round-trip rides per month
- Does not offer emergency transportation

Set Program Fee:
- $10 one-time application fee
- No fees for rides

Intake Process:

- Call or visit website to apply. Application and waiver form can be provided by mail or printed from the website.
- Ride reservations must be made by phone 10 days in advance. Cancellations must be made three days in advance.
