Description:
Provides in-home, rehabilitative behavioral health support services for youth 18 years and younger. Helps to decrease crisis episodes and hospitalizations. Provides a team of professionals to offer the following services:
- Intensive therapy
- Case management
- Psycho-education support
- Mental illness self-management
- Development of interpersonal skills
- Development of practical living skills
- Identification and use of natural supports
- Identification and use of community resources
Eligibility:
- Must be Lake County youth 18 years of age and younger who are experiencing psychiatric instability
- Must demonstrate a medical necessity for this service based on clinical assessments and treatment plans
