Description:
Provides care coordination for children under the age of 21 with special healthcare needs who are enrolled in a contracted Medicaid HealthChoice Illinois plan. Provides assistance with:
- Developing a care coordination plan
- Accessing diagnostic tests
- Finding specialized medical care
- Insurance assistance (All Kids and Medicaid)
- Paying for certain medical expenses
- Communicating with doctors, specialists and schools
- Preparing for transition
- Resources & information
Eligibility:
Must be children under the age of 21 with special healthcare needs who are enrolled in a contracted Medicaid HealthChoice Illinois plan. Eligible conditions may involve:
- Increased medical complexity
- Medically fragile
- Technology dependent
- Require in-home shift nursing
