Description:

Provides care coordination for children under the age of 21 with special healthcare needs who are enrolled in a contracted Medicaid HealthChoice Illinois plan. Provides assistance with:
Developing a care coordination plan
Accessing diagnostic tests
Finding specialized medical care
Insurance assistance (All Kids and Medicaid)
Paying for certain medical expenses
Communicating with doctors, specialists and schools
Preparing for transition
Resources & information

Eligibility:

Youth (birth-21 years) with an eligible chronic health condition
