Description:

Intervention/prevention services to students at-risk of dropping out of school. Services include assessment, referrals to outside agencies, mentoring, and case management.

Eligibility:

Must be a student in a Lake County school ages 6-17 years

Languages:

Spanish

Program Fees:

No fee

Intake Process:

Must be referred by school district. Concerned parties may contact the student's school district to request referral.

Documents Required:
- Schools and/or districts must complete a formal referral on website
- Copy of student's current attendance record
- Copy of documentation showing the school/district attempted different types of interventions (home visits, parent meeting, school district letter, etc.)
- Proof of sending a certified letter to the family with return receipt after the 10th unexcused absence
