Description:

Offers monthly financial support for township residents who cannot work due to health or medical condition. Provides support for rent, utilities, medical expenses, and other basic needs.

Eligibility:

Avon Township income eligible residents not eligible for public assistance programs

Call for appointment

Documents Required:
Birth certificates/citizenship certificate
Lease, most recent rent receipt, or mortgage statement
Social Security cards of all household members
Photo identification
Income information
Employment information
Documents on benefits client is receiving
Bank statements
Proof of marriage, divorce documents, or proof of separation
Copy of most recent tax return
Letter of acceptance or denial from other agencies
Proof of emergency
Disconnection or eviction notice
Written verification client is receiving or has applied for food stamps, TANF, medical card, unemployment benefits, SS/SSD/SSI, and child support
