Description:

Provides end of life care for the patient and family in the home or nursing home. Services include:
- Pain and symptom management
- Medication management
- Dietary and nutritional counseling
- Bereavement and pastoral care
- Medical social work
- Family counseling
- Hospice aides for personal needs
- Case management
- Physical, occupational, and speech therapy

Eligibility:

Must be a Lake County resident with a terminal or life-limiting diagnosis with a life expectancy of 6 months or less
