Description:

Offers end-of-life care for patients with terminal or life-limiting illness. Offers the following services for in-home or in-facility care:
- Pain and symptom management
- Medication management
- Case management and medical social work
- Physical, occupational, and speech therapy
- Family counseling
- Dietary and nutritional counseling
- Bereavement and pastoral care
- Hospice aides for personal needs

Eligibility:

- Must be adults 18 years of age or older with a terminal or life-limiting diagnosis and a life expectancy of six months or less
- Must be referred by physician

Program Fees:

- Fees vary based on service
- Accepts Medicare, Medicaid, and private insurance

Intake Process:

Call for assistance or physicians may submit referral by fax
