Description:
Provides in-home supportive services to older adults in Lake County. Aims to help seniors remain living at home or with caregivers. Coordinates and provides the following services:
- Congregate and home-delivered meals
- Transportation for grocery shopping and errands
- Housekeeping chores
- Personal care
Eligibility:
- Must be Lake County resident 60 years of age or older
- Liquid assets must be $17,500 or less for individuals or $35,000 or less for couples (Note: Assets are not the same as income; there is no income requirement)
Languages:
Interpreter services available upon request

Intake Process:
- Call to request services; An intake form will be provided via email
- Referrals can be submitted via email

- Proof of citizenship
- Proof of residency
- Proof of assets
