Description:

Provides vocational case management for adults with persistent mental illness. Offers job readiness, placement, and retention services for supported employment.

Note: Independence Center operates as a membership program. Membership requires that the individual has a persistent mental illness and Medicaid insurance to cover all fees. Members are encouraged to attend programs on a daily basis and contribute to the IC community.

Adults (age 18 years and older) with a persistent mental illness with Medicaid

Complete membership application online and call for appointment

Social Security card
Medicaid card
Referrals from other community agencies require current detailed psycho-social history, current psychiatric assessment, and signed release
