Description:

Provides comprehensive treatment services for adults with severe emotional difficulties. Offers the following three levels of care based on the individual needs of the client:
- Case management

- Shared Apartment Program - supported, transitional living

- Community Integrated Living Arrangements (CILA) - supervised, supportive living for individuals who have had at least three hospitalizations in a state-operated facility

Eligibility:

- Must be Lake County adults ages 18 years or older, with severe emotional difficulties
- For CILA Program, must have had at least three hospitalizations in a state-operated facility

Languages:

- Spanish
- Interpreter services available upon request

Program Fees:

Sliding scale fee based on level of income and number of dependents

- Accepts MCO Insurance
- Cash/Checks
