Description:
Provides a full range of services to individuals with severe mental illness. Services include:
- Evaluation
- Treatment Planning
- After Care Support
- Ongoing Case Management
- Life Skills
- Housing Assistance
Eligibility:
Lake County resident (age 18 or older), with mental health issues, and receiving Medicaid benefits.
Languages:
Spanish
Interpreter services available upon request
Program Fees:
Sliding scale fee.

- Accepts MCO Insurance
- Cash/Checks
