Description:
Offers free transportation services for seniors and adults with disabilities in West Deerfield, Vernon, Moraine, and Shields Townships. Provides assisted transportation and curb to curb service depending on client's needs. Clients can travel up to ten miles and allows for destinations outside of Lake County. Service can be used for shopping, errands, medical appointments, social events, or other needs.
Eligibility:
Seniors (60 years and older) or adults (25 years and older) with a disability
West Deerfield, Vernon, Moraine, or Shields residents (excludes North Chicago)

Program Fees:
No fee
Intake Process:
Call to schedule an in home intake appointment
Rides must be scheduled by phone at least five days in advance for assisted transportation and three days in advance for curb to curb services
