Description:
Offers intensive outpatient program (IOP) for adults and partial hospitalization program (PHP) for adolescents. Program includes daily lunch and transportation. Treats individuals with the following mental health disorders:
- Depression
- Bipolar disorder
- Anxiety
- Addiction
- Post Traumatic Stress Disorder (PTSD)
- Trauma from physical, emotional or psychological abuse
Eligibility:
Open to Lake County residents
- Must be 13-17 years of age for PHP
- Must be 18 years of age and older for IOP
Languages:
None
Program Fees:
- Accepts Medicaid and MediCare
- Accepts most insurance
- Offers sliding scale for uninsured patients (will work with patients to set up a payment plan)
