Description:

Provides in-home care to older adults and individuals with disabilities. Offers personalized care plans with certified nurse aids and caregivers, including:
In-Home care (personal, companion, and safety care)
Specialized care (including 24-hour services, end-of-life, in-facility, and post-hospital care)
Alzheimer's and Dementia care
Respite care

Eligibility:

Adults (age 21 and older)

Languages:

Spanish

Program Fees:

Set program fee
$38 per hour, accepts Medicaid, private insurance, and longterm care insurance
