Assists and processes first-time passport applications for adults, children, and adolescents. Does not offer passport renewal services.

Intake Process:

Call for an appointment. Appointments are accepted Monday through Friday 10:00 a.m. to 3:00 p.m.

Documents required:
- Completed DS-11 New Passport Application !€“ do not sign the application (Black Ink Only)
or
- Completed DS-82 Renewal Application - do not sign the application (Black Ink Only)
Forms may be downloaded at www.morainetownship.org/passport
- One Passport Photo or a $15.00 charge will be incurred
- Proof of U.S. citizenship (Must be: Certified copy of birth certificate, Certificate of Naturalization, or expired passport)
- Photo ID or driver's license
- Payment in check or money order only

If passport is for a child (under age 16), both parents must be present
