Description:

Provides community-based support services for adults with severe mental health and co-occurring needs. Staff members meet the client at home or in the community to help them fulfill goals in their case plan. Provides services such as counseling, accompaniment, medication management, and job assistance. Services are delivered by a team of mental health professionals.

Eligibility:

Must be Illinois resident 18 years of age or older with severe and persistent mental health disorder and at least one co-occurring need (such as homelessness, justice involvement, or substance use disorder)
