Description:

Provides transition coordination and support to individuals with mental illness. Prepares and helps them moved from state-funded facilities into permanent supportive housing. Following support services are available through the drop-in center:
Case management
Drop-in center
Peer support groups
Recreational activities
Computer access
Information and referrals to community resources

Adult (age 18 and older)
