Description:
Offers connections to permanent housing for people with serious mental illness or co-occurring disorders. Additional services include:
- Behavioral health treatment
- Peer recovery support services
- Employment assistance
- Integration with primary healthcare
- Health insurance enrollment
- Information and referrals to community resources
Eligibility:
-Must be 18 years or older
-Must be homeless or at-risk of homelessness in Lake County.
-Must have a serious mental illness or co-occurring disorder.
