FREMONT TOWNSHIP HALF-FARE TAXI PROGRAM:
Township has contracted with American Taxi to provide fare taxi coupons for Seniors and Disabled Residents. The goal of this program is to provide transportation to areas that our Dial-A Ride buses do not service. Coupon books are purchased at the Township Office for $15.00 and are worth $30.00.
Coupons are non-transferable. 1 book limit / month- unless medical necessity.

ROAD TO RECOVERY TRANSPORTATION REFERRAL:
Volunteer drivers donate their time so that patients can receive the life-saving treatments they need.

ELDERCARE:
Provides seniors with free rides to the doctor and grocery store.

CATHOLIC CHARITIES:
Seniors (60 & OLDER) Transportation Grant
Providing rides to Lake County seniors (60+) to attend medical appointments. They ask that the senior contact them 5-7 business days prior to their appointment to request services. Transportation is provided Mon-Fri but not on holidays. There is a $6.00 donation asked for each round trip provided.

Program Fees:

Set program fee:
- $3.00 each way for trips under 10 miles.
- $6.00 each way for trips over 10 miles.

Intake Process:

Filled out application, must be on file with township.
Rides must be scheduled 24 hours in advance
No change given back on coupons.
Can bring up to three riders at no additional cost.

Road To Recovery®
- It can take several business days to coordinate your ride, so please call well in advance of your appointment date.

ELDERCARE:
- Must call for an appointment (10 days in advance). They do not charge for the service but appreciate a $10 donation.
