Description:
Intervention/prevention services to students at-risk of dropping out of school. Services include assessment, referrals to outside agencies, mentoring, and case management.
Eligibility:
Must be a Lake County district student between the ages 6-17

Intake Process:
Referrals received from school districts.
Documents Required
- Schools and/or districts must complete a formal referral on website along with:
- A copy of documentation showing the school/district attempted at least different types of interventions (Home Visits, Parent Meeting, School District letter, etc...),
- Proof of sending a certified letter to the family with return receipt after the 10th unexcused absence and a copy of student's current attendance record.
