Description:

Provides telehealth appointments for medicine, physical therapy and behavioral health. Offers accessible healthcare to uninsured and under-insured adults in Lake County.

Offers primary care services for basic acute illness, vaccinations, and chronic-care issues such as diabetes, high blood pressure, or heart disease. Patients are seen by an inter-professional student team made up of medical, physician assistant, nursing, pharmacy, physical therapy, or podiatry students, and treated by a supervising healthcare provider. Information on low-cost pharmacy formularies and discount drug programs, also provided.

Also offers behavior health services, such as:
- Anxiety
- Depression
- Relationship Issues
- Eating disorders & food issues
- Weight management
- Pain disorders and pain management/issues
- Stress management
- Adjustment to medical illness or disability
- Family and social support issues
- Anger management
- Medication compliance
- Trauma

Note: Referrals to nearby free clinics for diagnostic testing and other supportive services, also available.
