Description:

Provides complete care to children from birth to age 18. Services include:
- Developmental assessments
- Hearing and dental screening
- Basic vision testing
- Immunizations
- Lead testing

Eligibility:

Open to all Lake County residents

Languages:

Spanish
Interpreter services available upon request

Program Fees:

Fees are assessed based on the services needed, with adjustments made depending on the individual or family income.
- Accepts most insurance
- Accepts Medicaid
- Accepts Medicare
- Accepts Children's Health Insurance Program (CHIP)
- Sliding scale fee based on income, if no insurance, must apply

*No one is denied services due to inability to pay
