Description:

Provides telehealth appointments for medicine, physical therapy and behavioral health. Offers accessible healthcare to uninsured and under-insured adults in Lake County.

Offers primary care services for basic acute illness, vaccinations, and chronic-care issues such as diabetes, high blood pressure, or heart disease. Patients are seen by an inter-professional student team made up of medical, physician assistant, nursing, pharmacy, physical therapy, or podiatry students, and treated by a supervising healthcare provider. Information on low-cost pharmacy formularies and discount drug programs, also provided.
