Description:

Provides information and referrals to older adults and their caregivers to local resources. Staff assess needs and offers referral to a range of services, which include:

- Care coordination services to help develop a plan of care
- Long-Term Care Ombudsman Program
- Legal Services
- Transportation Services
- Employment Information
- Nutrition Program (Home-Delivered Meals and Group Site (Congregate) Meals)

Eligibility:

Individuals looking for information for older adults. To receive services, older adults must be 60 years of age and older.
