Description:

Provides home visits twice a month to families with children birth to three years old. The goal is to prepare families for their children!€™s transition to preschool. Also offers age-appropriate activities for parents to repeat at home with their child. This includes communication, motor, self-help, social, cognitive, vision, and hearing.

Eligibility:

Children from birth to 3 years of age and their families and must meet income eligibilities

Program Fees:

No fee.

Intake Process:

Call or visit during program hours to apply or be referred by iGrow from the Lake County Health Department.
