Provides children up to age 21 with care coordination to access:

- Diagnostic tests
- Specialized care providers
- Answers to your insurance questions
- Resources and information
- Care coordination plans
- Communication with your child!€™s providers, school and others involved in your child!€™s care
- Transition support
- Financial support is available to families who qualify

Offers eligible children support with:
- Therapy
- Specialized equipment
- Transportation related to medical appointments and treatment.

- Blood Disorders
- Cardiovascular Impairments
- Craniofacial and External Body Impairments
- Eye Impairments
- Gastrointestinal Impairments
- Hearing Impairments
- Inborn Errors of Metabolism
- Nervous System Impairments
- Orthopedic Impairments
- Pulmonary Impairments
- Urogenital Impairments

Eligibility:

Must be families residing in Illinois with children who have a chronic illness or physical disability.

No fees/charges

Intake Process:

To apply download and fill out application located on the website and email to: dsccenrollment@uic.edu or you may mail it. You may also receive assistance over the phone or arrange for someone to meet you at your home or other location. Applications are available in English, Spanish, and French.

Once submitted, you will receive an Authorization to Release Health Information
