Description:

Provides financial assistance to help residents experiencing financial hardship. Offers assistance to pay for shelter, food, utilities, medical services or transportation.

Eligibility:

Must be resident of Shields Township and be within income limits. Aide is available once in an 12-month period

Intake Process:

Call for an appointment.

Documents required:
- Drivers license or State ID
- Social security card for everyone in the household
- Documentation from the Illinois Department of Human Services (if applicable)
- Documentation from Social Security Administration or the Department of Unemployment (if applicable)
- Current checking and savings account statement or copies of receipts or money orders that paid your bills for the past 30 days
- Pay stubs for the past 30 days
- Lease with landlord's name, address and phone number
- Current mortgage statement (if applicable)
- Doctors note stating that you cannot work (if disabled)
- Medical and doctor's records stating your medical condition
- Marriage or divorce papers
