Description:

Provides curb-to-curb transportation to older adults and people with disabilities of West Deerfield Township. Priority given to medical appointments, but non-medical transportation may be available. Participants must be able to get to and from van without assistance of driver. Caregiver may ride with participant. Offers up to two round trips per week.

Taxi-Cab Coupons:
Provides taxi cab coupons for seniors and adults with disabilities. Coupons may be used for one way rides to and from Highland Park Hospital and are valued at $4.25.

Eligibility:

- Must be residents of West Deerfield Township
- Must be older adults age 65 and over or individuals with a disability

Intake Process:

- Visit website to apply, or call to request application by mail
- All applicants with a disability will be interviewed to review safety procedures and protocols
- Once registered, call to schedule a trip. Medical appointments can be scheduled up to 30 days in advance; non-medical appointments needs at least 3 days in advance.

- Photo ID (driver's license or state ID)
- Copy of current utility bill
