Provides a list of hospice location for those living with serious illness across in the Chicago region. Provides a focus on quality of life and relieving the symptoms , such as:
- Pain
- Loss of appetite
- Nausea
- Anxiety

Eligibility:

Must be individuals with pain or other symptoms such as nausea, loss of appetite, difficulty sleeping, shortness of breath, fatigue, and/or anxiety resulting from a serious illness and have a prognosis of 12 months or less
