Description:

Provides comprehensive care for individuals who are in the final stages of their serious illness. A clinical team is available 24/7 to carry out the patient's individualized plan of care. Offers pain and medication management. Also assists with activities of daily living and personal care needs. Emotional and spiritual support is also offered for the patient's family. Hospice care can be administered in a nursing home, hospital, hospice facility, or in your own home.

Eligibility:

Must be individuals who have been diagnosed with a life-limiting illness that is progressing in spite of curative treatments and have a certified prognosis of six months or less.
