Eligibility:

Birth records:
The person named in the record (if at least 18 years of age)
The parent of the person named in the record
A legal guardian who provides proof of court appointment
An agent with expressed, notarized authorization (must contact our office to obtain the required form).
*Exceptions to these restrictions apply to those interested in Genealogical Research.

Marriage Record:
A marriage record may be requested by:
those named in the record,
child of person named in the record,
a family member, or
another individual acting as an agent or legal representative of the person named in the record.

Death Record:
If you are requesting a death certificate as the duly authorized agent or legal representative, you must submit a written document naming you as one of the following:

A licensed attorney acting on behalf of a decedent or his/her estate;
An agent authorized by power of attorney;
A court-appointed personal representative, executor/administrator, or
An agent with expressed, notarized authorization.
