Description:

Provides transition coordination and support to individuals with mental illness. Prepares and helps them moved from state-funded facilities into permanent supportive housing. Following support services are available through:
- Daily case management - provided through the Assertive Community Treatment (ACT) Program
- Psychiatric care
- Medication management

- Drop-in center- provides non-clinical Support (New Life Recovery Center - Zion & New Hope Recovery Center - Waukegan)
- Peer support groups
- Recreational activities
- Computer access
- Information and referrals to community resources

Eligibility:

- Must be an adult over the age of 18.
- Must have chronic mental illness.
- Must have resided in a nursing home for at least 6 months.

Interpreter services available upon request
