Provides ongoing mental health care and crisis care for adults, children and families by psychiatrists, psychologists and counselors. Services include:
- Anxiety disorders
- ADHD
- Behavioral and conduct disorders
- Depression
- Learning disabilities
- Counseling
- Medication
- Psychological testing.

Also offer outpatient services through Telehealth Services for Behavioral Health patients

- Any co-pay your insurance company requires you to pay for an office visit. Please the toll-free customer service number on your insurance card prior to your first visit to find out. Co-pay, is due at the time of your visit.

Intake Process:

Call for appointment.

Documents Required:
- Photo ID
- Current health insurance card
- list of any medications currently taking. This includes prescription or over-the-counter medicines, as well as any vitamins, minerals or dietary supplements.
- A list of any questions or concerns you would like to discuss with the doctor during your visit.
- A completed patient registration packet.
