Description:
Offers individuals and families opportunities to learn about conditions such as:
- Diabetes
- High blood pressure
- Obesity
- Healthy eating
Also provides referrals for other services and resources, makes connections to medical providers, and advocates for better health care. Provides monthly health screening options with medical providers. Offers peer to peer education with Community Health Workers
