Provides rehabilitation services for youths:
- Mental illness self-management
- Skill building
- Development and use of adaptive and compensatory skills
- Identification and use of natural supports
- Identification and use of community resources
Eligibility:
- Must be Lake County resident
- Youth experiencing emotional and behavioral instability in the home
- Must demonstrate a medical necessity for this service based on clinical assessments and treatment plans
